time of reenactment, your $5.00 per month fee may be used towards
the purchase of a lotion.
- The amount of your chosen membership will be deducted from
your CHECKING, CREDIT or DEBIT CARD on a monthly basis.
- The billing date is the 2nd of each month.
- The CUT-OFF DATE is on the 30th of each month. If you wish to cancel, freeze, or change your membership, simply come in and
sign the appropriate form.
- Today's purchase will include your FEE + any pro-rated amount until your next billing cycle.
